8/31/2023 EnchantaVaughn Approved During morning arrival, client was observed displaying a poor attitude while coming into the academic setting. The teacher mentioned to counselor that client has been experiencing some difficulty with adhering to instructions. Teacher also stated that he was experiencing frequent disruptions in the classroom when the client refused to comply with rules. Client failed to remain active during morning tasks and found himself falling asleep often. Client is constantly waken up by his teachers so that he doesnât miss a lot of his work. Once client falls behind he becomes extremely aggravated and gives up easily. Client finds himself off track mostly when itâs a writing or reading subject. Client has shown little interest in his school work and completing his assignments on time. Client lack the skills needed to perform at his full potential and this is a huge concern to all staff involved in his academic and behavioral treatment. Client is challenged and encouraged to stay awake for as long as he can. Client struggled to interact during morning academic hours, as evidenced by him not participating in class. Client behaviors were viewed as defiant and disrespectful. Client had a hard time transitioning into the first activity of the day. Client stated he did not want to be in school and that he wanted to go home. Client appeared tired and upset. Client had a difficult time making it his encore classes and needed a break. Client was taken to get a drink of water to regain his energy. Client does well once heâs out of the classroom environment. Client enjoys being around other kids that struggle in school because he doesnât feel like an outcast to the rest of his peers. Client talks a lot about his sleeping patterns and why they are off. Client explains he struggles going to bed at night. Client want to do better in school because he know his mom is counting on him. Client tells his behavioral specialist that he tries his best to stay up but itâs hard. Towards the end of the day client struggled to interact with his peers appropriately as he was observed being seated alone. Client appeared to be distant as he was seated away and given a bouncing ball to help him maintain his attention and be more involved in his school work. Client dismissed without incident but did not achieve objectives or meet expectations, as noted.
